killed [TRIANGULATED] filter 9.5/15 spread ±0.5 signals: 3 independent
What is this?
PA-Call Ledger is a lightweight tool where the biller submitting a prior authorization writes a 2-3 line rationale (why this should approve: pre-auth status, frequency limits, medical necessity narrative, payer-policy version cited) at submission. The payer's PA decision, returning in 5-15 days, resolves each prediction. Manager sees calibration by biller × payer × procedure × rationale-pattern: which billers cite stale frequency rules, which payer-policy updates the team missed, which medical-necessity narratives systematically fail at which carriers. AE's adversarial autopsy grades the rationale prose (not just approve/deny) — surfacing premise-conclusion severing ("covered code" ≠ "covered for this patient this year"), cosmetic confidence ("usually approves"), and lifecycle-state drift (rule that promoted last quarter now demotes). Volume of 20-50 PAs/month per 5-30-person practice keeps double-entry tractable; one recovered PA covers a month at £300-£600/location.
Why did we consider it?
PA-Call Ledger applies AE's rationale-grading and calibration infrastructure to a domain where reality grades itself in days, automation vendors target the wrong layer, and one recovered PA pays for the month.
What breaks?
Red team not yet run.
What did we learn?
Engine verdict: KILL (SKIP). Geography-confused, distribution-mismatched, transfer-learning-unverified product targeting a workflow regulators are explicitly automating away.
Filter scores
Five axes, each scored 0-3. Three independent runs by different model perspectives. Median shown.
| Axis | What it measures |
|---|
| data moat | Does this product accumulate proprietary data that compounds? |
| 10x model test | Does a better model make this more valuable, or redundant? |
| fast feedback loops | Can outputs be graded against reality in <30 days? |
| solo founder feasible | Can a solo operator build and run this without a team? |
| AI providers cant eat it | Do hyperscalers have structural reasons NOT to build this? |
Composite median: 9.5 / 15. Graduation threshold: 9.0. IQR across runs: 0.5.
Evidence
Signal A — Primary source
Prior authorization (PA) is a utilization management tool that enables plans to implement patient-focused goals of safe and appropriate medication use.
Signal B — Competitor with documented gap
Dentrix provides checklist-level PA creation and status tracking for dental procedures but has no rationale-capture field, no biller calibration scoring, no payer×procedure×rationale pattern analytics, and no feedback loop connecting PA outcomes back to submission quality.
Signal D — Demand proxy
{"found":true,"summary":"LinkedIn data shows 74% of dental practices report PA-driven workflow disruption, CMS is actively proposing new PA rules with dental implications, and multiple RCM advisory sources frame PA inefficiency as a top revenue-cycle pain point — all indicating strong latent demand for better PA outcome tracking in dental.","sources":["https://www.linkedin.com/posts/zentist_dentalbilling-priorauthorization-dentaldso-activity-7437557970923802624-WD2E","https://www.drbicuspid.com/dental-practice/office-management/insurance/article/15823127/cms-proposes-prior-authorization-rule-w…
