Add v2_a12 forward_clock_testability axis

What is the proposed change?

Insert v2_a12 axis after a11. Score 0-3 based on whether candidate's central value claim can be falsified within 90 days of a hypothetical solo build start. Mapping: 3 = explicit testable outcome with a named metric + horizon ≤90d; 2 = testable metric but horizon 90-180d; 1 = testable but horizon >180d or metric vague; 0 = no testable outcome stated. Reuses forward_clock pattern from S186.

Expected effect

On rerun of 43 graduated candidates, fewer than 8 score 3 (most candidates lack testable horizons); the 1 DEFER override scores ≤1. Composite spread of 2-3 points between testable and non-testable cohorts.

Falsifier — what would prove this wrong?

If ≥30 of 43 candidates score 3 (axis non-discriminating) OR the DEFER candidate scores 3 (axis fails on its design case), remove axis.
